Occupational Therapy Services

Services are provided by a licensed Occupational Therapist and are individualized based on client needs. Participation in services is voluntary and requires completion of intake procedures and informed consent documentation.

 

Nature-Based Services

Some services take place in outdoor or community environments. By participating, you acknowledge and accept the inherent risks associated with natural settings, including weather, terrain, and environmental conditions.

 

Appointments and Cancellations

Appointments will be scheduled in advanced. We request at least 24 hours notice for cancellations or changes. However, we understand this may not always applicable and so we ask to inform us as soon as possible if you have to cancel.

Payment

All payments must be made in full at the time of service unless otherwise agreed upon.

We are a private pay business and do not bill or process claims through insurance providers. Payment is the sole responsibility of the client at the time services are rendered.

Upon request, we can provide a superbill for services received. A superbill is a detailed receipt that you may submit to your insurance provider for potential reimbursement. However, we do not guarantee that any services will be covered or reimbursed by your insurance company. Coverage and reimbursement are determined solely by your insurance provider.

Communication

Communication via phone, email, or text is intended for scheduling and general communication. These methods may not be fully secure, and use of them indicates your acknowledgment of this risk.

 

Limitation of Liability

Resilient Roots OT is not liable for injuries, damages, or losses that may occur during participation in services, including those conducted in outdoor or community settings, except as required by law.

Participation in services is at your own risk.
